assure that children, youth and local communities have choice of treatment providers
Regional medicaid MH TX providers are in a revolving door, and our medicaid children, youth and families have only that option. Setting them up with TX with these providers does more harm than not providing TX at all. They need choice, access, equity, and time to trust.

Measure outcomes of programs it funds to see if they are effective. Suicide hotlines don't work well
Suicide hotlines do not work well. They are often busy and put you on hold or cut you off. They should be staffed specifically for people who are suicidal by skilled ppl including consumers who have experienced suicidal thoughts. High risk groups should also target certain diagnoses like borderline personality disorder which has a very high rate.

What you mean by prevention will kill kids. We need research into non-drug alternatives
Research futher Open Dialogue, Aaron Beck's cbt for schizophrenia, chadwick's cbt for psychosis and Acceptance and committment therapy. ALL non drug interventions. and look for others and research. STOP KILLING US.

Since faith-base is the 1st place a person seeks help, more efforts should be focused in that area.
The culture of many populations begin and end in there faith. More funds should be desinated for training in this area.

3 approaches to a national strategy for reducing underage drinking and adult problematic drinking
I like the inclusion of objectives on preventing alcohol problems, reducing both underage drinking and adult problematic drinking, and addressing the issue with both local efforts and a coordinated national strategy. After having coordinated community efforts to reduce adolescent substance use in an Illinois county, I recognize the value of local strategic prevention efforts, but that the strength of local efforts are limited by unfortunate national cultural influences of alcohol. I also recognize that adults establish many of the cultural attitudes and behaviors regarding underage drinking. Create Detox programs available to Substance Abuse and Mental Health Clients
The choise to be free of illegal drugs and psychiatric medications should be made available at all clinics and facilities of the Public Mental Health System of Care. All Providers need to be trained to utilize detox methods and drug free treatment modalities.

Mental health and substance use disparities are complex, intractable social problems that often defy precise definition and transcend policy & services. SAMHSA's SI needs to include innovative initiatives to work across systems and sectors, engage these communities, disseminate information on innovative practices, and provide training/TA on best practices/EBPs.

Given the high prevelance of trauma, focus on preventing child sexual, physical & emotional abuse
The vast majority of people diagnosed with mental health and substance abuse issues are trauma survivors. The best prevention would be prevention of violence, particularly violence against children.

We need effective school-based substance use prevention programs in elementary and middle schools.
I know the evidence for D.A.R.E was not impressive, but the number one risk factor to developing an addiction is age of first use. Once a child gets a taste for a drug it gets more hard-wired in a growing brain. A stitch in time saves nine. We need to focus more on pushing back that first use. Maybe a peer-based program would be more effective.

Encourage the use of traditional healing methods and use of Eastern medicine techniques .
promoting the use of FREE, effective traditional healing methods in the community would provide early help to those in need especially trauma recovery.
